Sometimes it has come from good management of the income one has-—being frugal instead of throwing money around.
I was accused of being wealthy a couple of years ago.
WHY? Because I OWN my property free & clear.
I also OWN my vehicles-—which I have had for 41 & 37 years respectfully.
Don’t smoke
Don’t drink
Don’t gamble
Don’t spend $$$$ on entertainment-—cell phones—jewelry—travel-—or foolish impulsive items.
NEVER made more than $65,000 in any year.
Best I can dig up - average annual wage in Washington State is around $57,500.
Ten times that is $575,000.
Oh well - a lot of hospital managers are going to have decisions to make.

If I wanted to work more...I could make $150k plus a year. During the COVID....I was offered lot's of money. But declined it..I work 3 12's a week max.
