Looks like it was a lipoma of some sort and it looks to have been surgically removed.

Whoah! It is like some deflated sac now. Maybe there was some large mind controlling parasite in there before.
Weight loss and eye brows are a little more Star Trek, but seems like it’s likely the same guy.

Yep, it looks like a lipoma. Inn the second pic you can see there is still a slight bump. Probably excess skin.

Both appear to be him.

Although the angles are slightly off for an exact comparison, the ears look the same and that’s something apparently difficult to look alike at random. The fluid/fat sac has gone down considerably along with his weight, as well as a little of the fat that was in his face.
