I read a fascinating article recently on Waksal that tracked his professional career, and demonstrated that he had been fired from every job he held for fraud, scientific misconduct, and generally bizarre behavior involving lying and deception. Maybe someone can provide a link?

At each level, his employers did a C.Y.A. when providing recommendations/comments to each successive employer, allowing Waksal to rise to the level he did.
