India and Pakistan would take different sides. Terrorists all over the world would see an opportunity to strike. North Korea would probably go south. South America could just stay out of it and become stronger, less dependent on the US, and more unified.

Yup, Spain and what passes for militaries in Eastern Europe would just scare the border guards silly...till the Russian boy scouts (yes they are boy scouts now) reinforced them...but their will might be broken by the mighty Italian hordes. As for the US...I highly doubt the US public would swallow the 5-10 million casualties it would take...that was about what the Wehremacht took. It wouldn't be a bad day when 10 soldiers died, that would be the average ten minute toll.
