Usually it is someone who is sick and cannot physically get to Mass yet with a sincere heart wants to receive Christ.
It could also be a Catholic woman who married a protestant in a civil ceremony before returning to the church whose husband was recalcitrant in having the marriage blessed so as to be sacramental.
