It is more than that. It is blatant dishonesty. Emphysema is a bad disease but it takes decades to kill you and if a person quits smoking when diagnosed all damage comes to a full stop. The damage done is not reversible but progression goes back to the normal levels for the vast majority of those that have it. If he only needs an inhaler then he is in NO WAY terminally ill unless he is smoking a couple of packs a day between puffs on the inhaler.
