Two authorizations by Pelosi, one according to the US Constitution & one ‘by law.’ Two swearings in. One on the Bible. One with who knows what words or actions?

No, that is incorrect. The Constitution only requires that the oath be said. It does not require that oath be taken on a Bible, or any other religious book.

The Bible is usually present due to a tradition started by George Washington. However that tradition has no force of law.

The second swearing in met the Constitutional requirements for the oath. The lack of a Bible was only a matter of bad taste. It had no legal effect on the oath.

There's no requirement to swear the oath on a Bible, or any other Holy Book. There's not even a requirement that the oath be taken orally, or that the Chief Justice administer it. Several President's have been sworn in by other than the Chief Justice, LBJ for example when JFK was killed, and a couple of others who took office under similar circumstances. Some of those re-took the oath later. One originally was not even done by a federal judge at all.

Why not? A Bible isn't required-- not every President has used one.
