One would hope that the aftermath of this would include patient, relentless, and thorough investigation and appropriate disciplinary action for all the malfeasors involved.

Just about every doctor I've ever seen gives me a copy of my "superbill" when I leave, with the diagnosis and the charges for the visit.
The supervisors should demand to see those, and not just these bogus "excuses from mommy".
