Brandon Lee was killed by a gun that was assumed to be loaded with blanks while filming The Crow.

If I recall correctly, the gun was loaded with blanks, since they were filming a scene where Brandon got shot multiple times. However, the same gun had been used in a previous scene where dummy rounds were in the chamber for the close up (it was a revolver), and the slug from one of the dummy rounds dislodged from the casing and went into the barrel. When the blanks were put in and fired, the blast sent the slug into Lee. At least, that's the official story. If true, the weapons safety guy was an incompetent or careless moron.

IIRC, the round had been lodged in the barrel the day before when the other scene was shot and the weapons guy neither checked nor cleaned the weapon over night.

You’re right. The gun was actually loaded with blanks. The bullet that killed him was lodged in the barrel from an unnoticed squib load from a previous scene. The combination of the squib and the blank made it a loaded gun.
