I'm five years younger than Obama, and was born in Mississippi, and my old microfiche long form looks very similar to the Obama long form document. Even the verbiage in many places is identical. Block 12b on the HI form and block 11b on the MS form both say "Kind of Business or Industry". Verbiage in HI block 6c is identical to MS block 1c. HI blocks 3 and 4 are identical to MS blocks 5a and 5b.
The Mississippi form does have some more information, such as previous pregnancies by the mother, length of pregnancy, birth weight, and if the mother is married. The Mississippi document does not have a place for occupation of mother, nor birth hour.
My daughter was born in 1992 in Montana and she received a long form birth certificate. So not that long ago did states still issue long forms
