Any other form would clearly NOT be an "original birth certificate".
Also your WND link was posted 2009 and indicated the certificate may not exist at all. This newer post looks like they still have their doubts.

Hawaii has not destroyed any original Birth Certificates and if they somehow did, what law authorized them to do so?
Send them a UIPA request and get it from the horse’s mouth if you don’t like what you read.
