Yes,they are enforced when the offenders are caught in the act,whether in public, or private via complaints from witnesses/neighbors or while a warrant for something else is being carried out.
Just like any other law.

Also the number of people arrested for committing sodomy in states that had laws prohibiting it was undoubtedly miniscule unless other crimes were involved.
Breaks my heart, it does, all those sodomy practioners arrested! Jailfuls of them!
