If you do these things while I’m paying for your health coverage, you can bet your ass I have an issue with it because you’re costing me money that, had you made more responsible choices with your health, I would not otherwise have had to pay.

Which is EXACTLY why the government should NOT be in the health insurance industry. Or any other industry for that matter. There are precious few things that the government can do, which cannot be done better and in a less costly manner in the private sector.
