I sincerely hope that there is a place for free form text on the questionnaire because my only two answers will be "Go to hell" and "Kiss my ass".

If your Doctor asks about you having guns in the home ask what training he has received & from where ?

If they don’t have serious training ask them if the Doctors malpractice insurance carrier knows they are practicing outside their scope of practice & would the Doctor mind if you told the insurance carrier.

That should basically shut down the questioning.
