Several months ago I received an apparently anonymous survey from Medicare that I promptly tossed because I just generally don't like to fill out questionaires. Then, SURPRISE, about a month and a half ago, I received a follow-up with the same survey, so I opened it to see what it was all about. I'd not received anything like it in the five year I've been on Medicare.

Cutting to the chase, I opened it this time. The form, that had electronic pixel coding on it like an airline ticket, was in two parts. The first was a fairly mundane-appearing customer satisfaction survey about my relationship with my primary physician and how he ran his practice. The second part, however, was really disturbing in that it asked for just about all the details of my personal medical history. What really got to me was the form did not request my name, address, or Social Security number. IOW it was designed to look like an innocuous, anonomyous form when it was the very opposite, as clearly shown by the electronic coding and the fact that they sent me a follow-up when I'd not returned the first form.

In light of this article, the other thing that really disturbs me now is I am a TN CCW holder, have been since 1995, and I purchased a .22 MP rifle in the last few months in a legally registered sale. Could all of this be related and why is Social Security trying, on an allegedly "random" basis, to learn all of this highly confidential, personal medical information with the fraudulent representation that it is not personally traceable and identifiable to the survey's responodents?
