They had to operate on his neck. My husband's carotid artery on his left side was 97% blocked. They had to operate to unblock it. That meant a neck operation. During the operation, the nerve that operates the left vocal cord was nicked.

As a result, my husband had no voice. He could not speak for three months until a voice specialist operated and put a substance on the left vocal cord so it would artificially meet with the right vocal cord. That is how he got his voice back.

His neck was in a pristine condition when they went in to clean out the carotid. This bomber's neck was shot up by a bullet even before they went into the neck. It's very possible he cannot talk due to damaging that vital nerve required to speak.
