There doesn’t seem to be any conspiracy here, only Duncan’s dishonesty and poor decisions.

The woman's family insisted that she had malaria. Duncan denied having contact with anyone with Ebola--how is that a lie, since he didn't know?

The lie was one of omission. Duncan opted to keep his exposure to Ebola to himself. He could/should have provided this critical piece of information to the nurse at triage in the first hospital visit. Because he refused to disclose his exposure, he gambled with his life.

His loved ones are not to blame for not knowing.

Coming from a region with Ebola spreading wildly, he had to have suspected. It would have been in his own self interest to have pressed the matter in the first visit. Families seem to fear being shunned more than being wiped out. They are reluctant to admit to the possibility of exposure even though they would be better off self quarantining.

Psychology isn’t my strong suite.
