Smoking increases your risk for virtually ALL cancers. Carter has been a non-smoker so long that the cause of cancer is not from smoking; it’s a geriatric disease of old age. Liver cancer is usually not a primary cancer.
I'd have guessed, not exactly wildly, brain cancer. Slow onset, probably began as Governor... or earlier.
