My take is that what Trump meant to say, but it came out "wrong" through elision, was something along the line of, "How stupid does Ben Carson think the people of Iowa are?"
That just seems to fit given the context and how he's saying it.
But as a great saying goes, "risk is the price you pay for opportunity."
