Under what scenario does the person without health insurance who cannot otherwise pay get treatment that doesn’t cost the rest us?

There isn't one. So there seem to be only three possible solutions. Force them to insure themselves, as Obama wanted to do. Treat them regardless and have taxpayers and insurance premium payers foot the bill, as is done now. Or let them die, which seems to be what you are proposing.

My dad was a doctor and during the fifties much of our back yard landscaping was done in trade not to mention the beautiful 8mm rifle he got as payment.

My preference doesn't enter into it; being Canadian it's a settled matter up here. The question is what the U.S. prefers. A lot of people hate the idea of the first option. The third option is repugnant for most rational people. And option number two is the pre-Obamacare status quo which I assume a President Trump would take you back to. So you tell me, which to you prefer?

That’s certainly the goal. I’m not sure of the background of the Canadian system, but at some point your country decided to share the burden of healthcare costs, but we in the U.S. have made no such agreement.
