Here is the issue I have with your analysis—and that of much of medicine.
It is based on statistics—that is the overall effect on large populations.
However, within those populations the affects vary widely (perhaps on something like a normal curve).
With centralized decision-making on who gets help and who doesn’t the bureaucrat will always rely on statistics—since they have no easy way to determine who fits where on the normal curve.
So, from the point of view of the state smokers are bad risks—and that drives policy.
From the point of view of the individual (and whether they should get medical care) the state policy is madness.
These “bosses” have addresses. Just sayin.

In the US the CMS just has a different name for it. Medicare and Medicaid will pay Hospitals and providers based on the future health of the patient.
The future health of the patient depends on lifestyle choices over which the Hospital/Doctor have no control. The result will be that Hospital networks and physician networks will shift the patient load in order to maximize payment from CMS.
The providers are given incentive to treat those who are healthy and have healthy lifestyles and to not treat obese sexually promiscuous smokers.

Actually obese people live longer than the underweight, statistically. So it seems to me under these satanic provisions the underweight should be denied the care, neh?
My hubs went into serious chemo treatment for Acute Myelogenous Leukeumia weighing over 300 pounds. He survived the treatments when many of the beautiful young men on the floor turned into wastrels and died. They could not eat and they had insufficient fat reserves. My heart breaks for them.
Mr. 60 year old fatty made it :) Dropped 100 pounds in the process, of course.
Anyway, and obviously, we should pay for our own health care and pay for what we want done. We all know it. Socialized medicine is nasty to people.

My friend in CA has Kaiser insurance. She needed back surgery and her doctor refused to do it until she quit smoking for four months. Meanwhile he gave her all kinds of meds including morphine for the pain .

That is true but they shouldn’t be denied needed surgery as long as they understand the risks. My friend in CA was denied back surgery unless she quit smoking for four months, meanwhile taking many pain meds including morphine. Hope she doesn’t come out being addicted to worse than nicotine.
