No way the DNC allows Kaine to run in place of Hillary. The DNC gets to pick, and they will pick Biden.
If Hillary is incapacitated *after* the election, Kaine automatically gets the nod.
I don't think Bill will give it up that easily. If there's one thing that he wants more than money, it's a steady stream of fresh, clean young interns.
Now that early voting has begun, to withdraw would insert a bazillion legal problems into the process. Not that it couldn't be done, but it would be problematic -- and costly.
No debates happened in 1964, 1968 and 1972. Carter had one debate in 1980, Reagan had two. If she has a pulse, she can be on the ballots.

If she croaks before election day, the baton passes to Bernie Sanders. He ran in the Dem primary and received the second highest number of delegates.
Kaine did not run in the primary, and was only chosen by Hillary to be on the ticket. The people did not vote for him.
