After watching many times, I saw it too. It was from inside right pant leg and does not look like a shoe.
Oddly it looks like a long object gets ejected from the back of her pant leg. Higher up than the cuff. Does not look like the shoe but maybe. Did a heel fly off her shoe?
It wasn’t her shoe. I have no idea what it was, but my goodness do I smell something horrible from a long way off. I may have just had my appetite ruined.

The MIL has some neurologic disorder. Docs can’t decide if it’s Parkinsons or not. She wears these braces similar to polio braces, without them she she cannot walk at all. Maybe Hillary is wearing something like that and and one broke. It’d explain her going down face first like that.

After looking at the video several times it looks like her shoe is stripped off as her foot scrapes over the curb. Then a hand reaches down and grabs it up.

I saw it too. It’s really clear during the up-close, slow-motion segment. Small metal object falls to sidewalk from right pant leg, then bounces as she’s being moved forward from the post she’s been leaning on. After it landed, it more than likely was kicked by someone alongside her or behind her as they were loading her into the van.
If you look closely, you will see it is a hook that goes into an eyelet, linking lengths of fence. There is an identical hook near the top of the length of fence.
Please look to the second video, about the middle of the picture. What you see circled in the fifth video, just ahead and above of the woman’s foot, is a close up of that hook.
Hillary’s fainting belies her claims to good health. That is plenty of justification to doubt her ability to serve (over and above the mountains of evidence of illegal activities). When we are so desperate to spot something bizarre with Hillary (other than her cranial, criminal and moral issues), we veer into the territories of Glenn Beck and Alex Jones, cheapening the ‘Trump’ brand. By focusing on this, the presstitutes can quite easily point to conservatives as being ‘loonie toons’.
Turn up the audio and you can hear the metal piece hit the ground as it falls.
It looks like an item was dropped and fell onto her lower pants leg. Person on video right has something in his hand to inject her and dropped it to grab her?
It could have been her left testicle.
