A condom can act as a barrier for fluids but it does not eliminate all skin-to-skin contact. As a result, transmission of STIs such as herpes, HPV, syphilis, pubic lice, or scabies can occur even if a condom is used. Condoms are not always used correctly, and some are defective or become defective during usage..

Yes, that's true. A condom greatly reduces the probability, but doesn't eliminate it.

The adult film industry has their own monitoring program: every performer has to be tested periodically (monthly, or even more often). But, it's not foolproof, either.
