Most likely possibilities include valvular heart disease (valve replacement) or coronary artery disease (bypass)
He also had colon cancer.
That said....I knew a patient that was in his late 40's...Didn't smoke..appeared healthy..and fit. And he had CAD...and had a by-pass.
His one problem...was STRESS.
