Captains of U.S. Naval vessels are authorized to use force to defend their ships without having to clear it through the CoC if they believe the threat is real and imminent. Captain Rogers of USS Vincennes CG-49 exercised that authority when he shot down an Iranian flight 655 with 290 persons on board.

The Vincennes had been harassed by Iranian small boats earlier, so they already on alert for a possible attack. They tried to contact the plane & gave warnings 9 or 10 times.
The Iranians didn’t warn planes about the Vincennes being there. They should have advised different flight paths to avoid the area. They didn’t care.
