It does happen.

Folks who are new to the game often will get a few buckets of the different ingredients and play around with mixing. A lot of the "books" on the subject that are sold with the supplies often give you just enough information to blow your fingers off.

In the commercial fireworks industry, many safety precautions are taken that the casual hobbyist either does not take, or is unaware of. Accidents happen.

I work with a fellow who is active in the amatuer fireworks culture. If his house ever burns down, it will be a show that the firemen will be telling their grandkids about.
