What a freak accident — although why was the first failure, with the primer-driven bullet stuck in the barrel, not followed up on? Gun goes pop (not bang) and no bullet to be seen. It took two foul-ups to cause the shooting.

I've been around a few film & theater productions (nothing major, just low budget films and music videos.) People are lackadaisical about safety, and they're doing dangerous things. A lot of stunt people get killed or seriously injured. All the director cares about is getting the shot. All everyone else cares about is keeping the director happy.
