Actually this is a two jaw deformity that is not uncommon. Notice the sallow cheek bones and flat appearance of the middle face. This is maxillary hypoplasia or an upper jaw that has not developed forward and down. This combined with a excessive horizontal growth of the lower jaw create a gross dentofacial deformity. I would surgically advance the upper jaw (maxilla) and surgically “setback” the lower jaw to create facial harmony and a functional occlusion.
