This can only happen as an act of collusion by the deliverers and the receivers. It's too easy to detect a fake in those sizes. Only if it is “understood” that the testing protocols will be intended to be beaten and gone around, does the deal go forward. If both parties are honest, and demand and conduct stringent testing upon delivery, fake bars can't be passed. Assuming that a significant (undefined in advance) number of bars will be removed for lab-level destructive testing, before the contract will be finalazed and custody exchanged.
It's an inside job. The names are known. People go to jail. Or they should. But they are known.

That tells me the volume of the bar, but how does that measure the density of the bar? A bar of gold and am identical size bar of silver would both displace the same amount of water, would they not? (assuming of course that both bars sink)

There is some question about whether Archimedes could have measured the different volumes of water accurately enough to prove the royal crown maker was a crook.
