It is very easy to discern. Run a DNA test. XY chromosomes = male. XX chromosomes = female. No amount of operations, therapy, or treatments will change that.

Not always. There is Klinefelter syndrome, for example, and other generic abnormalities. Some times they are chimaeras where only part of their bodies is affected by an extra chromosome.
