A semi-truck driver slammed into a firetruck Thursday evening that was assisting with a car crash on a Virginia Interstate, killing one firefighter and injuring several others. Wendy Tapia, 34, was allowed to conditionally graduate from the FDNY's Randall's Island training academy in May 2013 - despite failing the required running test, New York Post reports.
