The worst I’ve ever seen was in a video where the operator of an engine lathe (metal) leaned in too close to the spinning chuck and workpiece. It wasn’t pretty and no Hollywood special effects could’ve made it seem so gruesome. Industrial accidents are some of the worst you’ll ever see or hear about, and having even a close call with one can be traumatic.

Knew a man who was killed by elevator while working in the shaft. The story I heard was that he was told that the power was off. It was not.
Legal question...who is responsible for this man’s death. Inventor, designer, builder, programmer, installer, maintenance, or deceased himself?
Was the robot fired for murdering a co-worker?
I’m thinking an array of infrared/heat sensors should have been in the original schematics. Even if there were...why was the dumbass working on a machine that was not totally powered off? I don’t do repairs on my circular saw when it is plugged in...for a reason.
