I don’t want to pop everyone’s buble here, but the two different forms were used for different states, some needed the certification, some didn’t.
This article says that the form without the certification is the one on file in all 50 states. Do you have information to the contrary?

Both of those forms are for Colorado. If you state this information, post a link to your source and back up what you say.
Show me the certification from ONE state that requires it, please?
Here's the problem:
Nancy Pelosi signed, under penalty of perjury, a version where she specifically certified that 0bama is Constitutionally qualified for the job.
Then SHE PULLED THAT DOCUMENT BACK, and signed a document that was MUTE as to his Constitutionally qualifications, and submitted that one instead.
There is absolutely no reason that a state that doesn't require the eligibility statement would require that it be omitted!
The most logical reason is that she wanted to avoid committing perjury. (Treason is apparently less of a concern in her mind, as anyone familiar with her voting record can attest)
The other thing? She had to have known in advance, or at a minimum discovered after signing the first version, that he was NOT qualified. Otherwise, she would have let the first version stand.
She knew. She knew. She knew. She knew. She knew. She knew. She knew. She knew. She knew.
And, in the name of partisan politics, she deliberately betrayed our country and our Constitution.
