This is the FIRST TIME I know of that ANYONE has found a SEVERAL HUNDRED NUMBER DISCREPANCY---an AT LEAST 741 number discrepancy---and I have followed it closely FOR YEARS!!!
I seriously doubt that long form BC's are issued from the nurses stations. (plural)
They would have to come from the administrative office (singular) of a hospital.
(Not to mention hospitals tend to have only ONE labor/delivery area)
Logic would therefore dictate the numbers would be in sequential order.

This is OLD NEWS ~ the dispute there was that there was a deviation in sequence of a few numbers between his and theirs ~ and whole logical castles were built on both the idea that Grandma took a form home and filled it out after baby Obozo was brought from Kenya to whole stacks of forms being stolen for practice.
The simple solution is that the forms were pre-printed and addresses applied, etc. then distributed to different areas in the same hospital, and to different hospitals, and maybe even to various midwives and doctors who helped home births.
That initial 'form number' would have little to do with the sequence of the births.
