Nobody says, "Mr. President Obama" or "Mr. President Bush," so she'd be "Madame President" to her face or "President Clinton" in the third person. What people say behind her back is their business. How they distinguish her from the previous President Clinton is something the press will work out.

FWIW, saying "I respect the office, but I don't respect her," either means you try to be civil out of respect for the office or it means nothing. Not being civil is anybody's right, but don't trot out the old chestnut about respecting the office, if you actually don't.

Fortunately for me, I don’t respect an office, president or otherwise. I only respect people who deserve my respect.

Offices don’t have feelings, so I don’t bother respecting offices. When an office is occupied by a crooked, grasping, greedy, lying, baby-killing sodomite, I just follow my gut.
