Aside from striker fired handguns, some semi-automatics like Sig Sauer and Heckler & Koch have a double action/single action trigger system.

My 226, before it tumbled over in the canoe, hated me. I finally learned to shoot with just the tip of my finger on the trigger.

No external safety. With a round chambered it takes a good amount of force to double action the first shot.

If it was a double action pistol, seems this poor woman had the hammer up as its hard to imagine a 2 year old with enough strength to yank a double action.
