Authorities in central Florida say a man with a group of people on a plane doing military-style parachuting has died after his reserve chute accidentally opened as he knelt near the cargo door, pulling him out.
The Marion County Sheriff's Office said in a statement that the man was pronounced dead by paramedics after falling 1,200 feet to the ground Friday.acebook

When parachutists fall 1200 feet, it is usually from a plane.
Why didn’t the reserve parachute save him?
Probably a streamer or some other form of partial deployment.
The fall didn’t kill him. Hitting the ground did.
yeah, it’s called a fast fall malfunction. A streamer with a partial opening, but not enough to slow for a safe landing. If a reserve chute opens or deploys inside an aircraft fuselage, it will immediately rip the aircraft open when the door is opened.

It could have been the fall.Same with those who jumped/fell from the WTC.Massive rise in blood pressure and heart rate....huge stress on the heart and the blood vessels of the brain--> fatal heart attack/stroke.
