I think so. I tried to enlarge it in my post, to better read it, but it still came out fuzzy.
Looking at
my post #20, the form looks like it's printed long-hand to look nice, and filled in hand writing on the blank lines.
By the way, ask school kids today, if they'[re learning how to read and write in long hand.
Very few are, and that's another way liberals are stealing their ability to read our history, as most of it is NOT typed or printed.
