I guess then by your defintion, Obama isn’t a narcissist since he doesn’t give a rat’s ass about anyone but himself.
We don’t really know if he feels empathy for others. He may feel empathy but just ignore it. He also doesn’t have the conduct problems usually found with anti-social personalities.
I don’t like diagnosing people without having met them, but pretty much everyone agrees that Hitler was anti-social. Stalin was more of a paranoid personality than narcissistic IMO.
