Patient dies in hospital after 'do not resuscitate' form is mistakenly put in his files (UK)

A patient who suffered a heart attack on a hospital ward died because clerical staff had mistakenly inserted a ‘Do Not Attempt Resuscitation’ form into his medical notes.

Peter Clarke was not treated by doctors after going into cardiac arrest as a nurse had spotted the form in his files and, even though it was blank and had not been filled in, told other ward staff he should not be revived.

The blunder emerged at an inquest into the incident at Derby Hospitals NHS Trust, where bosses revealed staff had been ‘routinely’ placing the forms alongside medical records before they had been correctly signed and witnessed by senior doctors.

Seriously, I feel bad for the person who made the mistake. Everyone goes through so much paperwork to do anything nowadays, it’s easy to miss stuff. When you’re under time pressure, you can’t always review everything as thoroughly as you’d like to..

To be fair only the most extreme of the extreme on the left in the USA have called for a monstrosity such as the UK has. Generally, what the left here wants is single-payer socialized medical insurance, not true socialized medicine.

That’s sweet of you. I feel bad for the person who’s dead, and his family.

And it sounds like putting the form in the patient’s file wasn’t a mistake, but procedure.

So they did not perform life saving aid because an empty form was in his file. Well sheesh, thats a pretty low standard.

I would bet the clerk was just doing what he was told to do. It was the higher up, and you KNOW they are not going to take responsibility.

That is no difference. They seek to nationalize the healthcare industry. The hospitals will still be owned by private individuals (for now) but the owners will have no control.

The govt will determine what healthcare workers get paid, what services can be provided, how many patients a doctor can see, how many surgeries a doctor can perform, whether a hospital can expand, what services they can offer, who receives those services.

One draft I found on the house server indicated a limit of $5,0000 in services per person per year.

Who knows, the form could have been put in the file after the person died as a way of covering up.

Did the nurse make an ASSUMPTION that the blank form would have been filled out by the patient, since it was in his file, and then advise not to resuscitate? Pretty big ass-umption.

I doubt there are many on the left who would stop short of everything socialized — it’s just that they know they have to push their programs in stages

Everywhere I went before my surgery last summer, they all wanted copies of my advance directives. They are scattered all over doctors offices and hospitals in New Hampshire.
There’s lots of things worse than dying.

This happened in England. Good chance the "clerical staff" wore burka's and couldn't read the English on the form.
