No, it's a behavior pattern that allowed this to happen, stupidity and arrogance by people who think they are smart because they have certificates in frames, etc..

It’s also a condemnation of the gullibility of the American people, who believed the assurances of the oilmen when they said nothing like this would ever happen, and if it did, they knew just how to fix it. We wanted to believe them because it kept us doing what we were doing - driving around as if the oil that powered our cars was a snap to obtain and should be as cheap as water.

Of course, what would we have done if they had told us that it was all a big experiment that trusted to luck to keep from destroying the Gulf? What would we have done if BP had said, “We don’t really know what to do if something catastrophic happens”?

With 3400 other platforms in the Gulf, how much longer til this happens again? Are we willing to pay the bill? Or just writing off the Gulf?

Deepwater Horizon blowout

After the Deepwater Horizon drilling rig explosion on April 20, 2010, the blowout preventer should have activated itself automatically to avoid an oil spill in the Gulf of Mexico. Underwater robots were sent to manually activate the mechanism’s switch, to no avail. As of May 2010[update] it is unknown why it failed.[2] BP representatives suggested that the preventer could have suffered a hydraulic leak.[3] Gamma-ray imaging of the preventer conducted on May 12 and May 13, 2010 showed that the preventer’s internal valves were partially closed and were restricting the flow of oil. Whether the valves closed automatically during the explosion or were shut manually by ROV work is unknown.[3]

Injection success rate is about 95% on dry land, at 5,000 feet beneath the sea who knows? Better work or Obama advisers are considering nuking it to seal it.
