IMHO, the problems are endemic to the institution.
The movie Article 99 came out in 1992, based upon fiction novel written in 1978 (The House of God, the Novel), describes many of the issues being posed, although those were set in medical facilities mirroring Mass General and Harvard Medical School grads.
Similar stories were told in the MASH movie and series, which were based upon the 1968 book by an Army Surgeon who wrote about his experiences in the Korean War.

I am sure the problems at the VA long predate GW Bush.
Everyone is so reticent to be charged with “bashing” “Veterans” that unless the sh&*^% hits the fan over something at the VA, the VA bureaucracy just keeps running on autopilot, and VA programs may come and go, but how the VA operates probably seldom changes very much.
The VA needs a stronger IG and one more directly in tune with the clients at VA facilities.
There is a Patient Advocate system in the VA with a PA office in each VA facility. The problem is the PAs report to the local VA facility’s director. In other words, the local director can be sure that dirt coming from the PA does not get any notice above them.
Instead, the PAs in the VA need to be reporting to and working for the VA’s IG, and the IG needs to be brought annually before Congress with a report that includes and summarizes what the IG has learned from the PAs and what the IG has found concerning those things and what the IG thinks the VA has or has not done in response to them. Such a system would have noted and lifted up the delays annually a long time ago and then there would be no question when they began and how long they have persisted.
Don’t forget, the VA’s budget, from what I hear was just doubled for next fiscal year...From 84 billion, to 154 billion...
That will buy a lot more malfeasance and inefficiency, and deaths for that matter...
At least a few more “desks” for the non-doctors in the VA to work at...
BTW, “work” for a desk jockey, in this case, is such a loose term...

Romney was right about the 47%, but not just because their votes were bought & paid for. For a majority of Americans, Pelosi’s comments will be the only explanation they ever get for the VA problems. I have no sympathy for Americans in general; they are getting what they deserve. The increases in crime in neighboring Newark NJ are a vindication of everyone who voted against Barack Hussein Obama; let “his peeps” pay the steepest price.
