All of the Personal Care facilities that I know of here in my area of Pa have multiple areas for “stages”.
You start out Personal/Assisted Living and if needed move to Nursing Care which is in the attached buildings right there on the property.
Same staff for ALL of it.

My 95-year-old grandmother moved out of her senior center and is living quite well with my uncle and aunt. We celebrated her 95th birthday with a car caravan with honking horns decorated with balloons and streamers. That would have never happened if Grandma was in lock down in the senior center.
