My wife and I have decided not to get the third booster because our Doctor's office does not handle the vaccine anymore and we don't trust the other locations, such as Walmart, Walgreens, CVS, Sam's Club or Publix, to store, properly prepare, and administer such a temperature and time sensitive vaccine.
Our Doctor's office directed us to go to https://www.vaccines.gov/ for vaccine locations. Doesn't that send a chill up your spine?
The only clinic on the list for our area is Japanese, the Kuraoka Clinic.
When people started using ivermectin for covid the fda or cdc came out (prob Fauci) and banned it in the USA. Then the patients and their doctors using it for malaria, rheumatoid arthritis, and lupus, who need it chronically to function, got so upset that the rule immediately changed thst ivermectin could continue to be used for these maladies but it was NOT SAFE FOR COVID somehow. What a crock.
Ivermectin shortens the lifespan of mosquitoes that ingest it. An endectocide and in that manner it interferes with the transmission of the malaria parasite. Ivermectin is effective on worms and blackfly larva but I’m pretty sure that it doesn’t kill the malaria parasite.

I was right there with you on everything but that.
In 1976, I was in the USAF at McClellan AFB and after a Commander's Call, Colonel Joska ordered the whole auditorium to stay after we were dismissed to get a flu shot. I went right back to duty in the Tech Control Facility in the basement of the Comm Group Headquarters. Almost ever year since then I have had a flu shot. When this Covid 19 came around, I was getting up in years, have multiple comorbidities, and to this minute I don't look forward to fighting off Covid 19 at my age.
I retired as a PMI® and ITIL® V3 certified Program and Project Manager from Verizon so I know all about risk management. I have no induced fear of Covid 19, and have stated many times that I knew I would survive it if I did come down with it.
You obviously have a great fear of the vaccine.
Just so you know, my brother who retired from the USAF, was fully vaccinated and later caught Covid 19. He checked himself into the BAMC clinic in San Antonio, was treated with monoclonal antibodies and bounced right back the next day. The PA he usually sees didn't even know if they could give the monoclonal antibodies, left the exam room to check and came back to tell him that he was a perfect candidate for it. My brother has a pacemaker and two metal hips, so of course he was. He said that monoclonal antibodies shot wrecked him, thought he would die for four hours, then the change was like the difference between night and day.

What jumped out to me on this story was the coersion to agree to a DNR which sounds criminal to me.
I personally would want to be kept on a machine in perpetuity with a flatline brain scan. DNRs are another Satan trick as far as I'm concerned. My dad had a DNR and died in 1979 at the age of 56 after his fourth heart attack, but only a few years later he would have had a quadruple bypass and might be still alive today. Or at least live longer than he did.
