After questioning the value of general mask-wearing early in the COVID-19 pandemic, the Centers for Disease Control and Prevention decided the practice was so demonstrably effective that it should be legally mandated even for 2-year-olds. A new review of the evidence suggests the CDC had it right the first time.
That review, published by the Cochrane Library, an authoritative collection of scientific databases, analyzed 18 randomized controlled trials that aimed to measure the impact of surgical masks or N95 respirators on the transmission of respiratory viruses. It found that wearing a mask in public places “probably makes little or no difference” in the number of infections.
These findings go to the heart of the case for mask mandates, a policy that generated much resentment and acrimony during the pandemic. They also show that the CDC, which has repeatedly exaggerated the evidence in favor of masks, cannot be trusted as a source of public health information.

Here is the study:
• Meta-analysis and systematic review finds that wearing masks in the community probably makes little or no difference to the outcome of influenza‐like illness /COVID‐19 like illness (laboratory‐confirmed or not) compared to not wearing masks; moderate‐certainty evidence (Cochrane Database Systematic Rev. 2023;1:CD006207. DOI: 10.1002/14651858.CD006207.pub6). The pooled results of RCTs did not show a clear reduction in respiratory viral infection with the use of medical/surgical masks. There were no clear differences between the use of medical/surgical masks compared with N95/P2 respirators in healthcare workers when used in routine care to reduce respiratory viral infection. The use of a N95/P2 respirators compared to medical/surgical masks probably makes little or no difference for the objective and more precise outcome of laboratory‐confirmed influenza infection (RR 1.10, 95% CI 0.90 to 1.34; 5 trials, 8407 participants; moderate‐certainty evidence). We included 11 new RCTs and cluster‐RCTs (610,872 participants) in this update, bringing the total number of RCTs to 78. Six of the new trials were conducted during the COVID‐19 pandemic; two from Mexico, and one each from Denmark, Bangladesh, England, and Norway. The RCT is considered the gold standard of clinical trials. Although this review only assessed the benefit of respiratory protection for the wearer, and not for preventing viral transmission to others it likely would have found the same...given that exhaling generally bypasses the filters on the mask as the air takes the path of least resistance out the sides and top.
