As an ex education employee…and a once avid fan of the novel/movie “A Clockwork Orange” a 16 year old can be a big kid or a hardened adult criminal in every way except his age.

The "medical episode" was probably the thug falsely claiming that he "couldn't breathe" as the thugs are now trained to do in order to claim that they are "victims". In a case like this where a severe beating is warranted, he is probably trying to protect himself from being "accidentally" dropped down a few flights of stairs (as he deserves) by the police as he is taken into custody.
