Enclosed please find a request for a verification in lieu of a certified copy for the birth record of Barack Hussein Obama II. In addition to the items to be verified in the attached form, please verify the following items from the record of birth:

I'm confused by part of the statement above:

"In addition to the items to be verified in the attached form...."

What "items to be verified in the attached form" is Bennett talking about, because I don't see any "attached form" on the internet.

In other words, I would like to see the "attached form" that Bennett is talking about above.

If someone could help me, I would appreciate it.

The request form that Bennett said he copied off the internet, filled out, and sent in with the $5 is at http://hawaii.gov/health/vital-records/pdf/birth.pdf . Itams requested to be verified include the gender, date of birth, city and island of birth, father’s name, and mother’s name. The only one of those that doesn’t relate to Presidential eligibility is gender. The only one that was included on the response Bennett got was city of birth.
