I’m thinking the guy stuck a magazine in the gun and didn’t know he had to jack a round into the chamber. “I don’t know why it didn’t fire, I put the bullets in it!”
Broken firing pin from dry-firing? I know certain small semiautos (.25s are notorious) have that problem.
Don’t ask me how I know that, but it took several weeks before my mother found her firing pin was mysteriously broken when she tried target shooting hers, lo, these many decades ago.
Boy, was she P.O.ed, but safety in numbers: I have 3 older brothers. ;-)
