I’m more confused than he/she/it is.
It’s a woman who had a few features surgically altered, and considers herself a man. If it was a man who got the surgery, he wouldn’t have the uterus neccessary for pregnancy. Aside from a few external features cut up and sewn, it’s still a woman.

I had to re-read it 3x to figure it out.
This particular woman had some masculinizing cosmetic surgery, but retained her female reproductive capacity. So she was able to get pregnant.
It's really not so hard to comprehend (though still nauseating) once you know the lingo.
